Area Port of New Orleans ports of entry will accept printed copies of CBP Form I-95.
The approved PDF version of the form may be downloaded from the CBP.gov website below.

Please note that the OMB Date is expired, however this form is still valid for use and is under review by OMB awaiting a new expiration date
