SUBJECT: REQUEST FOR APPROVALS OF CBP FORM 3499, APPLICATION AND APPROVAL TO MANIPULATE, EXAMINE, SAMPLE OR TRANSFER GOODS
Effective immediately, all requests for approval ofCBP Form 3499, Application and Approval to Manipulate, Examine, Sample or Transfer Goods should be forwarded to Chief Supervisory CBP Officer Michael Ruiz. The documents may be faxed to 504-623-6671 or emailed to MichaeU.Ruiz@CBP.DHS.GOV. If Chief Ruiz is not available, the documents should be sent to Supervisory CBP Officer Ronald Mora at the same fax number or may be emailed to him at Ronald.J.Mora@CBP.DHS.GOV.
